BRMC's Home Health Care has its administrative offices located across from the hospital at 1504 Clinic Drive (directly behind Taco Bell). Home Health Care Services may be provided to any client who meets homebound status, has a skilled need, and requires intermittent care following Medicare Guidelines.
Home Health Care Services are currently being provided to residents in Lawrence, Monroe, and Southwestern Brown, Eastern Greene, Western Jackson, Martin, Orange, and Northern Washington counties.
Scope Of Services
Bedford Regional Medical Center Home Health Care provides professional and paraprofessional services to clients who are under the care of a physician. A Doctor of Medicine, Osteopathy, Podiatric, or Dental Medicine must order the services for the treatment of a medical need. Skilled nursing services are provided by and/ or under the supervision of a registered nurse and in accordance with the Physicianís Plan of Treatment. Physical Therapy, Occupational Therapy, Speech Therapy, Medical Social services, and Registered Home Health Aide services are also available.
Hours of Operation
Normal operating hours are from 8:00 a.m. to 4:30 p.m. Monday through Friday. The central administrative office is closed on Saturday, Sunday, and Holidays. Physical Therapy, Occupational Therapy, Speech Therapy, Medical Social services provide services only during normal operating hours. Services are provided on an ìas needed basisî on the following holidays: New Yearís Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day and Christmas Day. The nurse on call or scheduled RN or Registered Home Health Aide provides services necessary on Saturday, Sunday, and Holidays. Services are available on a 24 hour/day, 7-day/week basis for skilled nursing by the nurse on call. After hours and on weekends, the nurse on call takes calls.
The admission process identifies whether the client is home bound, has a skilled need, requires intermittent care, and the care is reasonable and necessary. An OASIS data set is completed on all clients who are not pregnant and/or over the age of eighteen years of age. An OASIS is a group of data items designed for the purpose of systematic measurement of client Home Health Care outcomes. Assessment is an ongoing process. With ever visit, the home health care provider performs an assessment. Reassessment occurs with any significant changes in the clientís condition and specific intervals of time during their episode of care.
Continuity of Care
Case management occurs with each client. Each client is assigned an RN who is responsible for case management and continuity of care. Continuity of Care is a multidisciplinary approach and is reviewed weekly by the case manager.
